Additional Information:
1. Be prepared to spend approximately 2 hours for your procedure.
2. Please print ALL documentation above and complete each section (also
initialing each box indicating you understand and comply). Printing and
completing documentation will cut down the appointment time.
3. If you do not have access to a printer, SV Brow Design LLC will provide you with a printed copy at the time of your appointment, but requires you to read all documentation prior to your procedure.
